How to Use a Penis Pump, Step by Step

How to use a penis pump the right way. The steps, the pressure, how long and how often, and how to avoid side effects. Built on 31,227 hours of pumping logged in the PE Trainer app, our coaches and Baseem, who has bought over 50 pumps.

The short answer

To use a penis pump, trim your hair, get semi-hard and put a little water-based lube on the base. Seal the cylinder against your body and pump slowly to 3 to 5 inHg. Hold about 2 minutes, let all the air out, and repeat for 10 to 15 minutes. Start 2 to 3 days a week. Stop right away if you feel pain or numbness, or see a color change.

Then build up slowly. The typical man in our app ends up pumping at about 7 inHg, about 20 minutes a day. Harder and longer did not mean more girth. Steady weeks did.

Baseem Has Bought Over 50 Penis Pumps

Over the past few years I have bought more than 50 penis pumps. Most of them were trash. I also gained over an inch in length and half an inch in girth, using pumps and extenders together.

In one test, I pumped 10 to 20 minutes a day for a month on a basic electric pump. My erections got harder and thicker, and my girlfriend noticed. Did I gain permanent thickness in a month? Probably not. Nobody does. What a month gives you is better erections. Lasting girth takes months.

All those pumps taught me what matters. A cylinder that fits your girth. A gauge, so you know your pressure. Build quality that lasts for years. And a pump small enough to travel with. Those four things are what I built the Epic Pump around.

At the end of the day, consistency beats equipment.

What Baseem and our coaches tell new pumpers

  • Get a pump with a gauge. Never guess your pressure.
  • Get semi-hard before you go in. It seals better.
  • Start with 2 to 3 short sessions a week, 10 to 15 minutes each.
  • No more than 30 minutes a day, even when you are advanced.
  • Weaker erection the next morning? Take a day off.
  • Interval pumping, up and down, beats holding one pressure.
  • Measure on a rest day, never right after pumping.

What You Need Before Your First Session

A penis pump, also called a vacuum pump or vacuum erection device, has three parts. A clear cylinder that goes over the penis, a pump that pulls the air out, and a seal at the base. The pump creates negative pressure, a vacuum, inside the cylinder. That draws blood into the penile tissue, so the penis fills up and gets hard. Get these parts right and pumping is easy and comfortable.

  • A pump with a pressure gauge. A screen or a dial that shows inHg, cmHg or kPa.
  • The right cylinder. Wide enough for your girth, long enough for your length.
  • A soft pad or sleeve. It seals the base and keeps your testicles out.
  • Water-based lube. A thin layer on the base and the pad.
  • A timer. Most electric pumps have one built in.
  • A towel and 20 quiet minutes.

Pick the Right Cylinder Size

The cylinder size matters more than the pump. Too narrow and you rub against the walls and blister. Too wide and the pump pulls in skin, or even a testicle, and you swell more at the base. Too short and the head presses on the end.

PUMP 2 to 3 in of room past the tip 10 to 15% wider than your shaft Soft pad seals the base
How a cylinder should fitAbout 10 to 15% wider than your shaft, and 2 to 3 inches longer than your erect length. Up to about 8.5 inches erect, a 9 inch cylinder works. Longer than that, pick 12 inches.

To find your shaft width, measure your erect girth around the middle and divide it by 3.14. Then pick the cylinder just above that. Here are the sizes our cylinder sizing calculator picks for common girths.

Erect girthShaft widthCylinder
4.0"1.27"1.5"
4.5"1.43"1.75"
5.0"1.59"1.75"
5.5"1.75"1.93"
6.0"1.91"2.32"
6.5"2.07"2.32"
7.0"2.23"2.5"

Epic cylinders come in 7 widths from 1.5 to 2.87 inches. The typical man in our app starts at a 5.0 inch erect girth, which fits a 1.75 inch cylinder.

Snug cylinders favor length and wider ones favor girth, our coaches say. When you start to fill the cylinder wall to wall, it is time to size up.

Use a Pump With a Gauge

Pressure is the one setting that can hurt you. Without a gauge, you are guessing. Our coaches will not even talk about a pump that has no gauge. A digital screen is easiest to read, and a smart pump can hold the pressure steady for you.

We don’t promote pumps here without a pressure gauge

Chez, coach in our Discord

Lube, a Soft Pad and Trimmed Hair

Use a water-based lubricant. Petroleum jelly sticks inside the cylinder and is hard to clean. A thin layer on the base of the shaft and on the pad is enough. A soft silicone pad spreads the pressure, so you do not get the red ring at the base that hard plastic leaves. Trim the hair at the base short, so it does not break the seal or get pulled.

How to Use a Penis Pump in 10 Steps

This is the routine our coaches teach new members, and how Baseem pumps himself. Follow these instructions in order, especially the first few times. Good technique matters more than the pump. Your first session should take about 15 minutes.

  1. Step 1

    Trim and wash

Trim your pubic hair short around the base. Hair breaks the seal and gets pulled into the cylinder. Wash first. A warm shower also gets the blood moving.

  2. Step 2

    Warm up

Spend a few minutes warming up with a warm towel, a shower or a heat pad to increase blood flow. Our coaches also do a short first set at low pressure, about 3 inHg for 3 to 5 minutes.

  3. Step 3

    Get semi-hard

Go in semi-erect or fully erect, never fully flaccid. In his demo, Baseem gets semi-erect with a few seconds of light jelqing first. It seals better, and your shaft expands instead of the loose skin. Going in soft is a top cause of red spots.

  4. Step 4

Lube the base, add the pad

Put a thin layer of water-based lube on the base of your shaft and on the soft pad. Baseem's trick is to put the soft pad around the base of your penis first, then slide into the cylinder. In water mode, you do not need lube.

  5. Step 5

    Seal it against your body

Press the cylinder flat against your pubic bone. Keep your testicles out of the opening. A soft pad that fits stops them, and the skin at the base, from getting sucked in.

  6. Step 6

    Pump up slowly

Turn on the pump, or squeeze the hand pump slowly, and watch the gauge. Stop at your start pressure, 3 to 5 inHg in your first weeks. You should feel a firm pull, never pain.

  7. Step 7

    Hold, then release

Hold about 2 minutes. Then let all the air out with the release valve. Rest a few seconds to a minute, then pump back up. That is one set. A smart mode can do this for you.

  8. Step 8

Repeat for 10 to 15 minutes

Do 4 to 6 sets. Never stay in more than 10 minutes without a full release. Stop early if anything feels wrong.

  9. Step 9

    Release and slide out

Press the release button until the pressure reads zero, then slide out. Never pull the cylinder off while it still has suction. Rinse the cylinder and pad.

  10. Step 10

    Log it and check tomorrow

Log the session in the app. Next morning, check your erection. If it is weaker than usual, take a day off. That is Baseem's rule.

How Much Pressure Should You Use?

Start low. Use 3 to 5 inHg for your first weeks, and build up slowly over months. The typical man in our app settles around 7 inHg. Our coaches treat 10 inHg as the upper limit.

10hg is often what we tell people is the upper limit

Chez, coach in our Discord

inHg, kPa and cmHg Explained

Pumps show vacuum pressure in different units, and mixing them up is a common mistake. Inches of mercury (inHg) is what our coaches use. 1 inHg is about 3.4 kPa and 2.5 cmHg. So 8 cmHg is light, but 8 inHg is a strong pull. Check which unit your screen shows before you start. On the Epic Pump you can switch units on the screen and in the app.

What It Should Feel Like

Our coach Chez puts it simply. It should feel uncomfortable but not painful. If you feel pain or discomfort beyond a strong pull, lower the pressure. You will feel a strong pull and the shaft filling up, and the skin may go a bit red. Pain, numbness, tingling, a cold head or a blue or purple color are signs to let the air out right away.

What Pressure Men Really Use

In our app, the typical man pumped at 5.9 inHg in his first week and 7.1 inHg by week 13. After that, pressure stays about flat. Across all 94,005 pressure readings, 40% were under 6 inHg, 28% were 6 to 9 and 32% were 9 to 12. Almost nobody logs more than 12.

More Pressure Did Not Mean More Girth

We split 375 men who measured their girth months apart by the pressure they usually pumped at. Here is the share in each group who measured thicker than when they started.

Under 5 inHg53 men58%

The men at 5 to 7 inHg did as well as the men who pumped much harder. Our coaches say higher pressure mostly buys more swelling, and one of them found it on himself.

More than 9, for me, only causes more edema. Not more expansion.

How Long Should You Use a Penis Pump?

10 to 15 minutes a day to start, in sets of about 2 minutes. After a few weeks, build to 20 to 30 minutes, still in sets. Baseem and both of our coaches agree on a cap of about 30 minutes a day.

I wouldn't pump more than 30 mins a day split into 15/15 morning evening

Baseem, in our Discord

Baseem said the same thing in a video interview about penile rehab. He recommends about 15 minutes a day, or at most 15 in the morning and 15 in the evening, because past that the results do not get better. Men who overdo it can end up with weaker erections instead of stronger ones.

How Long Men Really Pump

A typical pump day in our app is 20 minutes. Half of all pump days fall between 13 and 30 minutes. 28% are 15 minutes or less, and 11% run past 45 minutes. A typical session on the Epic Pump lasts 12 minutes, so many pump days hold more than one session.

Longer Days Did Not Beat 10 to 20 Minutes

We grouped the same 375 men by how long a typical pump day lasted for them.

Men who pumped 10 to 20 minutes a day did at least as well as men who pumped 20 to 45. A short session you repeat for months beats a long one that wears you out.

Never Go 10 Minutes Without a Break

Whatever your total time, let the air out at least every 10 minutes. Long sets without a release are what cause dark discoloration, according to our coaches. Short sets of 2 to 5 minutes with a full release in between are safer, and they feel better too.

How Often Should You Pump?

Start with 2 to 3 days per week, with a rest day in between. Once that feels easy, you can go up to 3 to 5 days a week, as long as your erections stay strong. Think of it like any exercise routine. You grow on the rest days.

2 to 3 Days a Week Was the Sweet Spot

Men who pumped 2 to 3.5 days a week had the highest share who gained girth and the lowest share who lost it. Among men who kept going for 3 months or more with 20+ pump hours, the pattern held. 78% of those pumping 2 to 3.5 days a week gained girth, against 64% of those pumping less. The typical regular pumper in our app pumps 1.7 days a week.

Check Your Morning Erection

Your morning erection is the best guide to how much is too much. If you wake up and your erection is weaker than usual, your penis is tired. Take a day off and come back at a lower pressure. Baseem gives every new pumper this rule, and it works for extenders too.

Pump vs Extender, Which One Actually Builds Size?Baseem's beginner plan for a pump and an extender · 5 min

Is There a Best Time of Day?

No. Of 3,560 recent sessions in our app with a local time, 28% started in the morning, 23% in the afternoon, 27% in the evening and 22% late at night. Pick the time you can keep. Many men pump in the shower in the morning, or after their extender session at night.

Interval Pumping vs Steady Pumping

Steady pumping means you pump up to one pressure and hold it. Interval pumping means the pressure goes up and down, again and again. Most of our coaches and Baseem prefer intervals.

  • Manual intervals. Pump to your pressure, hold 2 minutes, release fully, pump right back up. Our coach Chez does 5 of those, a 10 minute break, then 5 more.
  • Smart intervals. An electric pump does it for you. It pumps up, holds a few seconds, drops the pressure and pumps up again, hands-free.
  • Why intervals. Each release takes the strain off the skin. Our coaches say that is what keeps discoloration away.

It is also how most men use the Epic Pump. Of the sessions where our app recorded the full pressure curve, 75% were interval sessions.

I really believe that interval pumping is the way to go.

A Weekly Plan With a Penis Extender

74% of men who pump in our app also use an extender. The extender builds length and the pump builds girth, so they work well together. Here is a simple week that fits both.

Weekly plan illustration: penis extender five days a week, penis pump three days a week, two rest days
5 extender days, 3 pump days, 2 rest days.

Pump Before or After Your Extender?

Pump after. Our coaches give three reasons. A swollen head is more likely to blister in the extender cup. Swelling loosens your tape as it goes down. And some men no longer fit the cup after pumping. If you also wear an all day stretcher, the order is extender, then stretcher, then pump.

In our app, 78% of days with both put the pump after the extender. The order did not change the result. Men who mostly pumped after extending gained +0.12" of girth in a typical case, the same as men who pumped first. So if pumping first is more comfortable for you, that is fine. Just rest in between.

Water or Air Pumping?

It is a matter of taste. No coach in our Discord thinks water gives better results, and Baseem pumps with air himself.

Air

Quicker to set up, works anywhere, quieter. Needs a little lube to seal. The Epic Air is air only and the quietest version.

Water

Warm water seals for you, so no lube. Nice in the shower or bath. The Epic Hydro does water and air.

I also don’t believe water pumping works any better than air

Chaswood, coach in our Discord

Epic Pump being used with water in a bathtub
Water mode works in the bath or shower.

How to Pump in the Shower or Bath

  1. Fill the cylinder with warm water, not hot.
  2. Get semi-hard, then slide in. Press the cylinder against your body so it seals.
  3. Pump slowly and watch the gauge. Stop at your start pressure.
  4. Use the same sets as with air. Hold about 2 minutes, release, repeat.
  5. Release the pressure fully before you slide out. Rinse and dry the cylinder.

On the Epic Hydro, the motor turns on and off to hold your pressure in water mode, and a safety release kicks in if the water goes past the 19 cm mark. Only the Hydro is made for water. Keep an air-only pump head dry.

How to Use a Penis Pump for ED

Penis pumps are a standard treatment for erectile dysfunction (ED). In the US, the FDA classes vacuum pumps and constriction rings as Class II devices for getting and keeping an erection (21 CFR 876.5020). Doctors suggest them to men who cannot take ED medication, or who want an ED treatment without pills. A pump can help you achieve an erection, and a ring helps you maintain the erection for sexual activity. For sex, the steps are the same until the end.

  1. Pump to a full erection, slowly, the same way as above.
  2. While the pressure is still on, slide a constriction ring from the cylinder onto the base of your penis.
  3. Release the pressure and take the cylinder off. The ring holds the blood in.
  4. Take the ring off within 30 minutes. Never sleep in it.

Using a Constriction Ring Safely

The constriction ring, also called a cock ring or tension ring, is what keeps you hard, so it is also the part that needs care. Use a ring made for pumps, in the right size, and never more than 30 minutes. A ring that is too tight hurts and can bruise. The penis can feel cool and look a bit blue with a ring on, and ejaculation can feel different. Remove it right away if it hurts or your penis turns numb. For enlargement training, do not use a ring at all.

After Prostate Surgery

After prostate cancer treatment, and a radical prostatectomy in particular, many men lose their natural erections at night for months, and the penis can shrink. Daily pumping is used in penile rehabilitation to keep blood flowing to the penile tissue, which protects erectile function, penis health and penis size. As the rehab expert in that interview with Baseem put it, think of the pump as a replacement for the night erections you are missing. Talk to your urologist about when to start.

What the Studies Say

Most research on vacuum devices is about ED and surgery. Short daily sessions, usually 10 to 15 minutes, helped men keep their length. In one more study, only 1 in 36 men who pumped on most days for 90 days after surgery lost 1 cm or more, against 48% in earlier studies (Dalkin 2007).

After prostate surgery

About 2 cm saved

Men who started pumping 10 minutes a day one month after surgery kept their stretched length. Men who waited 6 months lost about 2 cm at 3 and 6 months.

+0.8 cm more

One month of pumping 10 to 15 minutes a day before an implant added about 0.8 cm more stretched length than no pumping.

Free abstractJournal pagePMID 27654466

For penis enlargement in men with a normal penis size, the research is thin. A small study of men who pumped 20 minutes three times a week for 6 months found no real change in length (Aghamir 2006), and a 2025 expert review says the same (Wang 2025). Girth has barely been studied. That is why we look at our own app data, where steady pumpers gain girth over months.

Side Effects and How to Fix Them

Used the right way, a penis pump is safe and effective for most men. Most side effects come from too much pressure, too long without a break, or the wrong cylinder. Here is what men in our Discord run into, and what our coaches tell them.

Side effectUsually meansWhat to do
Swelling and a "donut" under the headNormal after pumping. Fluid collects under the skinLower pressure, shorter sets, a cylinder that is not too wide. A short piece of silicone sleeve under the head helps. It goes away in hours
Red spots (petechiae)Too much pressure too fast, going in soft, or no lubeStop for a few days, then come back at about half your usual pressure
BlistersCylinder too narrow, or pumping right after the extender cupSize up the cylinder. No training until healed. A hydrocolloid patch helps
Dark or purple colorToo long without a releaseRelease at least every 10 minutes. Use interval sets
Red ring at the baseHard plastic edge pressing inUse a soft silicone pad
Testicle or skin sucked inCylinder too wide, or no padRelease at once. Use a soft pad and the right width
Numbness or tinglingPressure too high for too longRelease now and rest until it is fully gone
Weaker erections the next dayOvertrainingTake 2 days off, then come back lower and shorter

One warning from Baseem himself. He once got blisters from pumping with a vibrator on the cylinder, because the vibration, in his words, "makes everything feel good and you don't realize that you're over pumping." If you use a vibrator, watch the gauge and the clock.

Signs to Stop Right Away

  • PainDiscomfort is normal. Pain is not. Release the pressure.
  • NumbnessTingling or no feeling means too much, too long.
  • Blood in your urineStop pumping and see a doctor.
  • A feeling you need to pee all the timeOne of our coaches got this from pumping above 12 inHg. See a urologist.
  • Bruising that growsStop and let it heal fully before you pump again.
  • Bumps or sores that look newThose are not from pumping. See a doctor.

When Will You See Results?

Right after a session you will look and feel bigger. That swelling fades within hours. Harder, fuller erections usually come within a few weeks. Lasting girth takes months of steady sessions.

Penis pump results timeline: swelling right after, harder erections in weeks, measured girth after months
Swelling first, better erections in weeks, real girth over months.
  • Right after. Fuller and heavier for a few hours. That is blood and fluid, not growth. Never measure now.
  • The first weeks. Many men notice stronger morning erections and better feeling. One verified buyer wrote, "After the first 2 weeks I wake up with morning wood bigger and thicker than I have had in many years."
  • Month 3 and on. Measured girth starts to move. In our app, the typical pumper had added about 0.20 inches of girth by month 12, and the top 10% about 0.63 inches or more.
  • 50 hours and up. 72% of men who logged 50 or more hours of pumping measured thicker, and 48% gained a quarter inch or more.

Men who message us after 12 to 16 months of pumping and extending report an average gain of 1.6 inches, length and girth combined, and often more when they stay consistent. In our app, the top 10% of men who did both for a year gained 1.6 inches or more combined.

Girth goes all the way around, so a small number in inches is a big change you can see and feel. See all the month by month numbers in our best penis pumps guide.

What Our Members Report

We read every girth update in our Discord where a member posted his own before and after numbers over 2 months or more. Across 16 men, the typical gain was about half an inch of girth (0.48 inches) over about 8 months, and half of them reported half an inch or more. Almost all of them pumped as part of a bigger routine, with an extender or hanging. It is a small group that chose to post, so treat it as a story, not a promise.

One coach adds a warning that every new pumper needs. Edema can make you think you gained, but it is temporary. Measure on a rest day, at the same time of day, with the same tape.

8 Mistakes That Hold Men Back

Most bad pumping experiences come from the same few mistakes. Avoid these and you are ahead of most men who try a pump.

  • Too much pressureMore pressure means more swelling, not more girth. Start under 5 inHg.
  • Long sets without a releaseLet the air out at least every 10 minutes.
  • The wrong cylinderToo narrow blisters, too wide pulls in skin. Measure first.
  • Going in softGet semi-hard first, so the shaft fills and not the loose skin.
  • No gaugeIf you cannot see your pressure, you cannot keep it safe.
  • Measuring right afterPost-pump swelling fools everyone. Measure on a rest day.
  • Skipping rest daysWeaker morning erections mean you need a day off.
  • Quitting in the first monthLasting change takes months. Most men stop long before that.

Most Men Quit Too Soon

That last mistake is the biggest one. Of 4,845 men who logged a pump session in our app, here is how many kept logging sessions for at least this long.

Still pumping after 1 week46%
After 1 month35%
After 3 months23%
After 6 months15%
After 1 year8%

Time between a man's first and latest pump session in the PE Trainer app.

Most men fail not because the tools don't work, but because they quit before the results arrive.

How to Clean and Store Your Penis Pump

  • After every session. Wash the cylinder and soft pad with mild soap and warm water. Rinse well.
  • Dry it fully. Let the parts air dry before you put them away, so nothing grows inside.
  • Keep the motor dry. Only a pump made for water, like the Epic Hydro, can go in water. Wipe an air-only pump head with a damp cloth.
  • No petroleum jelly. It sticks inside the cylinder and is hard to wash out. Use water-based lube.
  • Charge it. The Epic Pump charges with USB-C, and one charge lasts for many sessions.
  • Store it clean and dry. The box keeps the parts together.

How to Use a Penis Pump FAQ

How long should you leave a penis pump on?

Start with 10 to 15 minutes in total, split into sets of about 2 minutes. After a few weeks, 20 to 30 minutes is fine. Never stay in more than 10 minutes without letting the air out. A typical pump day in our app is 20 minutes. If you use a constriction ring for sex, take it off within 30 minutes.

How often should a man use a penis pump?

Start with 2 to 3 days a week. Build up to 3 to 5 days if your erections stay strong. In our app, men who pumped 2 to 3.5 days a week had the highest share who gained girth (70%) and the lowest share who lost it (8%).

Should you be hard before using a penis pump?

Be at least semi-hard. Going in soft makes it hard to seal and pulls in loose skin, which causes red spots. Baseem gets semi-erect with a few seconds of light jelqing before he starts.

How much pressure should I use on a penis pump?

Start at 3 to 5 inHg in your first weeks. The typical man in our app settles around 7 inHg after a few months. Our coaches treat 10 inHg as the upper limit. In our app, men who pumped harder did not gain more girth.

Do you need lube for a penis pump?

For air pumping, yes. Use a thin layer of water-based lube on the base of your shaft and on the soft pad, so the cylinder seals. In water mode, the water seals for you, so you do not need lube.

Can you use a penis pump every day?

Some men do, at low pressure and for short sessions. Baseem's cap is 30 minutes a day, split into 15 in the morning and 15 in the evening. Take a day off whenever your morning erection feels weaker than usual. In our app, 2 to 3 days a week did as well as pumping more often.

How long does a penis pump keep you hard or bigger?

The swelling right after a session fades within a few hours. For sex, a constriction ring keeps the erection after you take the cylinder off, for up to 30 minutes. Lasting girth only comes from months of steady sessions.

Does pumping increase girth permanently?

It can, over months. In our app, 72% of men who logged 50 or more hours of pumping measured thicker than when they started. The typical pumper had added about 0.20 inches of girth by month 12, and the top 10% about 0.63 inches or more. Results vary.

Can a penis pump make you longer?

Not by much on its own. In a study of men who pumped 20 minutes three times a week for 6 months, length did not change in a way that counted (Aghamir 2006). Pumps are better at girth and erection quality. For length, an extender is the tool with the studies behind it.

What size penis pump cylinder do I need?

Pick a cylinder about 10 to 15% wider than your shaft and 2 to 3 inches longer than your erect length. For example, a 5 inch erect girth fits a 1.75 inch cylinder. Our cylinder sizing calculator does the math for you.

Is it better to pump with water or air?

Neither works better for results. Our coaches and Baseem all say water does not beat air. Water is nice in the shower or bath and needs no lube. Air is quicker, quieter and easier to do anywhere.

Should I use a penis pump before or after an extender?

After. Our coaches pump last because a swollen head can blister in the extender cup, and swelling loosens the tape. 78% of same-day sessions in our app put the pump after the extender. Girth gains were the same either way, so pick what is comfortable.

Why do I get a donut after pumping?

The donut is fluid that collects under the head of the penis. It is common and goes away within hours. Use less pressure, a cylinder that is not too wide, and shorter sets. Never measure until it is gone.

What are the red spots after pumping?

Small red dots are tiny burst blood vessels. They usually come from too much pressure too fast, going in soft or not using lube. Stop for a few days, then come back at about half your usual pressure.

Who should not use a penis pump?

Get medical advice first if you take blood thinners, have a bleeding problem or sickle cell disease, have heart disease, or have diabetes with less feeling down there, which raises the risk of injury. The same goes for recent surgery or other medical conditions in that area. Stop and see a doctor if you see blood in your urine or feel lasting numbness.

Can I use a penis pump with ED pills?

Many men do, but ask your doctor first. In one trial after prostate surgery, men used a pump and a daily ED pill together (Zhang 2022).

How do I clean a penis pump?

After every session, rinse the cylinder and soft pad with warm water and a little mild soap, then let them air dry. Keep the motor head dry unless it is made for water. Do not use petroleum jelly, which sticks inside the cylinder.
