Does Jelqing Work? What the Science and Real Men Say
Jelqing is free, so it is the first thing most men try. Here is what studies, urologists, our coaches and our own data say about it, the risks nobody mentions in the videos, and what to do instead if you want real length or girth.
146K subscribers on YouTube. Inventor on US Patent 11,918,505. I tried jelqing years ago and stopped. We make extenders and pumps, so read this knowing we are biased too. That is why we show data.
The short answer
It has never been proven. No study has ever measured a size gain from jelqing. We searched a database of millions of research papers and found only 7 that even mention it. None measured length or girth. Some men on Reddit say it added a little, usually a fraction of an inch, measured by themselves. But about 1 in 3 of the 89 men we read there got hurt, most often with weaker erections. Doctors and our own coaches tell men not to do it, because you cannot see how hard you are squeezing.
Our data points the same way. Of 363 men who logged hand exercises in our app, the typical one did just 16 minutes in total, only 13% were still doing them 6 months later, and 82% moved on to a device. If you want length, an extender is the at-home method with trials behind it. If you want girth, use a pump with a pressure gauge.

What Is Jelqing?
Jelqing is a hand exercise that is supposed to make the penis bigger. You make an OK sign with your thumb and index finger around the base of a half-hard penis, then slide it slowly toward the head, over and over, for 5 to 20 minutes. It is also called penis milking, because the motion looks like milking. Fans say it builds both length and girth, and it costs nothing. That is why so many men start here. In our quiz, 1,749 men said jelqing or hand stretching is what they had tried so far, more than 1 in 5 of the men who had tried anything.
There is no official way to do it. The Sexual Medicine Society of North America notes that because jelqing is not a proven technique, there is no standard process. So every website, video and forum teaches its own version, with different grips, speeds, numbers of strokes and levels of hardness.
Where Jelqing Comes From
The word is often said to come from an Arabic word for milking, and the method is often sold as an ancient Middle Eastern ritual. Nobody has been able to prove that story. A 2022 history talk at the American Urological Association meeting described jelqing as the best-known way men have tried to stretch the penis. What we know for sure is how it spread. Men found it on early internet forums and in paid e-books in the early 2000s. One member of our Discord remembers finding it around 2002, "when it was like an e-book". Today it lives on YouTube, TikTok and Reddit.
How Jelqing Is Supposed to Work
The idea comes from the gym. Lifting weights causes tiny tears in a muscle, and the muscle heals back bigger. Jelqing fans say that squeezing blood toward the head of a semi-erect penis does the same thing. The extra pressure is supposed to stretch the spongy penile tissue that fills with blood during an erection, cause microtears, and make it heal back larger. Some also say it improves blood flow and erection quality.
Muscles grow from strain. Spongy erectile tissue does not.
The problem is that the penis is not a muscle you can train. It is made of spongy erectile tissue inside a tough outer sheath called the tunica albuginea. The tissue fills with blood when you get hard. The muscle inside it is smooth muscle, not skeletal muscle like your biceps, and it does not grow from strain. As urologist Dr. Fenwa Milhouse puts it, "the penis isn't quite like our muscles."
Jelqing vs Manual Stretching
People mix these up, so it helps to be clear. Jelqing means squeezing and sliding along a half-hard penis to push blood toward the head. Manual stretching means gently pulling a soft penis out and to the sides, with no squeezing. Manual stretching is a common warm-up. Baseem does a minute or two of it before his extender. Jelqing is the one that worries doctors, because it puts force on a penis that is full of blood.
Jelqing
Half-hard penis. OK grip that squeezes and slides from base to head. Pushes blood into the shaft. Force is hard to judge.
Manual stretching
Soft penis. Gentle pull straight out and to each side, held for a few seconds. No squeezing. Used as a short warm-up.
The Claimed Benefits of Jelqing
Websites that sell jelqing programs promise a long list of benefits. Here is each claim next to what is actually known.
Claim
What is known
A longer penis
No study has measured it. Traction is the method with length data.
A thicker penis
No study has measured it. The thicker look after a session is temporary swelling.
Harder erections
Some men feel this for a while. Too much force can do the opposite and cause ED.
Better blood flow
It pushes blood in during the session. No study shows a lasting change.
Lasting longer in bed
No data. Pelvic floor exercises are the tested way to build control.
Fixing a curve
No data, and it may make a curve worse. Traction is a tested treatment for Peyronie's disease.
Why Baseem Stopped Jelqing
I tried jelqing years ago, like most guys do, because it is free. It felt like it was doing me more harm than good. I also went looking for research with a decent number of men behind it, and I never found any.
The big problem is control. You grip under the head so it does not slip, and that grip can easily be too tight. Then you squeeze and pull, and there is no way to know how hard you are pressing, or how hard you even need to press. There are dozens of jelqing exercises online, and nobody knows which ones help and which ones hurt. You cannot control the direction either, and pulling the wrong way can change the shape of your penis.
That is why I went the other way. With an extender, you can see the force you are using. Studies have tested it, and thousands of men have used it. Today my routine starts with a minute or two of gentle hand stretches, then the extender for an hour or two, then 15 minutes of interval pumping. Everything is logged in the app, so I know it is working.

Does Jelqing Work?
There is no scientific evidence that it does. Not one study has measured a man's penis before and after a jelqing program. Every claim you read comes from men describing their own results, usually without a careful measurement. Here is what the research and the doctors who treat penis problems actually say.
What the Studies Say
We searched Europe PMC, a free database of millions of research papers in medicine. Only 7 papers even mention the word jelqing. None of them tested it. The few that mention it say three things: men try it often, men report poor results, and men sometimes get hurt.
We looked for a study that measured jelqing gains. There is none.
Survey, 90 men
Tried it, poor success
Men anxious about their size were more likely to try jelqing, pumps or stretchers, and they reported poor success. Up to 80% of them had tried something like jelqing.
The International Consultation on Sexual Medicine found only stories from men, no clinical data. It adds that penile injuries from it are not uncommon.
Reviewers found that exercises and most other non-surgical methods are not backed by science. They called extenders the only evidence-based way to lengthen the penis.
Compare that with traction, the other way to stretch the penis. Penile traction devices, also called extenders, have been tested in clinical trials. Here are two of them.
54 men, 6 months
+1.2 cm erect length
Men wore an extender 4 to 6 hours a day. Erect length went up 1.2 cm on average, and it held 3 months after they stopped.
Trial devices were worn many hours a day. Results vary. Links go to the free abstracts on Europe PMC.
What Urologists Say
Urologists are the doctors who see men after a PE injury, and they are close to unanimous. Dr. Rena Malik, a urologist with millions of views on YouTube, says there are no good studies on jelqing. She describes one tiny study, run by a pump company, that found no change in length in 7 men. Doctor Eric, another urologist, puts it simply.
There are no scientific studies that support jelqing.

Does Jelqing Increase Girth?
Many men try jelqing for girth, and it is easy to see why. Right after a session, the penis looks thicker and heavier. That is blood and fluid pushed into the tissue, and it fades within hours. No study has measured lasting girth from jelqing.
If you want girth, a pump does what jelqing tries to do, with a vacuum you can measure and control. In our app, 66% of men who logged 10 or more pump hours measured thicker months later, with a typical gain of +0.12 inches. Among men who logged nothing at all, 41% measured thicker, which is simply how much measuring wobbles. With 50 or more pump hours, 72% gained, typically +0.24 inches. Every girth method is compared in our guide on how to increase girth permanently.
Does Jelqing Increase Length?
No study shows that either. Length is harder than girth, because it means slowly stretching the tough sheath around the erectile tissue. That takes a steady pull for hours a day, for months. A few minutes of strokes is not the same thing. This is exactly what an extender does, and why it is the method with trials behind it.
Is Your Penis Size Already Normal?
Before you try anything, it helps to know where you stand, because most men who jelq do not need to. A review of studies where doctors measured up to 15,521 men found the average erect penis is 13.12 cm, about 5.17 inches long, and 11.66 cm, about 4.59 inches around (Veale 2015). In our app, the typical man measured 6.26 inches bone-pressed, which is higher partly because men measure themselves and press into the fat pad. The Sexual Medicine Society of North America points out that most men who worry about size have a perfectly normal penis. A true micropenis is rare.
Why Some Men Swear It Works
Some men really do believe jelqing made them bigger, and they are not lying. There are honest reasons they see a change.
Better erectionsPushing blood into the penis can make the next erection feel firmer. A firmer erection measures a bit longer.
A fuller hangRight after a session, the soft penis hangs lower and looks thicker for a few hours.
Measuring after a sessionSwelling adds size on the tape. Measure on a rest day and you often see the gain disappear.
Weight lossLosing belly fat shows more of the shaft. One member in our Discord gained size while losing weight, then lost it again when he gained weight back.
Other methodsMany men jelq while also pumping, hanging or using an extender, then credit the jelqing.
Wanting it to workMen who put in months of effort expect a result, and small measuring errors look like growth.
What Our App Shows About Hand Exercises
The PE Trainer app is our free training log. Men can log manual sessions, which means jelqing and stretching by hand, next to their extender and pump sessions. So far, 363 men have logged 897 hours of hand exercises. That gives us something no study has: a look at what men actually do when they try it.

Most Men Quit Within a Month
Jelqing is free and needs no gear, so you might expect men to stick with it. They do the opposite. Only 42% of men who logged a hand exercise session were still logging them a week later, compared with 60% for the extender. A month later, it was 31%. By 6 months, only 13% were still at it, half the rate of extender users.

8 in 10 Move On to a Device
296 of the 363 men who logged hand exercises (82%) later logged an extender, pump or hanger session. 104 men logged hand exercises before anything else. So for most men, jelqing is not the plan. It is the free first step before they buy a tool.
Most men who start with their hands end up on a device.
It also means we cannot measure jelqing results in our own app. Only 3 men used just their hands and measured themselves twice, which is far too few to say anything. Almost everyone who keeps going adds a device.
What Hours on a Device Do
Here is the contrast. These are men who measured their bone-pressed erect length months apart, grouped by what they logged in between. Even men who logged nothing show a little "gain", because measuring is never perfect. That is the baseline. The share of men who measured longer climbs with hours on an extender.

Some Men Say It Worked, a Little
Reddit is kinder to jelqing than our Discord. 51 of the 89 men say they gained size. But look closely. Only 31 of them gave a number, and only 6 said they measured bone-pressed length. 42 of the 89 also used something else at the same time, most often hand stretching or a pump. The typical claimed gain was +0.7 inches of length (25 men) or +0.39 inches of girth (19 men). Several said their gains came in the first months, then slowed or stopped.
Maybe I was just a good responder but jelqing increased my girth like crazy.
31 of the 89 men reported an injury. The most common was not bruising. It was weaker erections or ED, in 14 men. Injuries also came early. 10 of the 31 got hurt in their first month, and 6 in their first session or first few days. Of the men who got hurt, 12 say they recovered, 5 had not when they wrote, and the rest did not say.

So the most generous reading is this. Jelqing may add a little girth for some men, early on, and the price is a real chance of hurting yourself with a force you cannot see. A pump gives the same filling effect with a gauge on it. In our app, 72% of men with 50 or more pump hours measured thicker.
Is Jelqing Safe? Side Effects and Risks
Jelqing is not the most dangerous thing men do for size. Injections and surgery carry bigger risks. But it is not harmless, and the videos that teach it rarely mention the side effects. A 2025 study of penis enlargement videos on YouTube found that they seldom mention the risks, such as scars, bruising or nerve damage.
Side effect
What happens
Pain and soreness
Squeezing a half-hard penis strains the tissue. Common after too many strokes.
Bruising and red spots
Small blood vessels under the skin break. Red or purple dots, or a bruise along the shaft.
Skin irritation
Friction, especially dry jelqing without lube, rubs the skin raw.
Numbness
Too much force can stretch the nerves on top of the penis. It usually comes back over weeks to months, and rarely takes up to a year.
Erectile dysfunction
Damage to the blood vessels can stop the penis from trapping blood, so erections get weaker.
A bend or curve
Scar tissue from small injuries can bend the erect penis, which is called Peyronie's disease.
Hard flaccid
A penis that stays rubbery and semi-firm when soft, often with pain. The cause is not proven.
From the SMSNA patient page, the 2024 consensus on penile augmentation, published case reports and urologists' videos. No study gives a rate for any of these.
Can Jelqing Cause Erectile Dysfunction?
Yes, it can. Dr. Rena Malik describes a study of 37 men seen at one clinic over 11 years who got erectile dysfunction after jelqing. Their scans showed the penis could no longer hold blood in well enough to stay hard. Doctors call this a venous leak. She also describes a man whose jelqing led to ED and a penile deformity. Too much force can injure the nerves too. Dr. Stefan Buntrock of UroChannel explains that overstretching a half-hard or erect penis can cause numbness and weaker erections, which usually recover over weeks to months, and in worse cases up to a year.
Can Jelqing Cause Peyronie's Disease?
It may. Peyronie's disease is scar tissue in the sheath around the erectile tissue, and it causes curvature of the erect penis. It often follows small injuries that happen when the penis is full of blood, which is exactly when men jelq. The Sexual Medicine Society of North America warns that these stretches may raise the risk of Peyronie's disease and erectile dysfunction. If you already have a curve, do not jelq. Traction is actually one of the treatments doctors use for Peyronie's, because it sets a steady, controlled pull. Read our guide on penis extenders for Peyronie's disease.
What Is Hard Flaccid Syndrome?
Hard flaccid is a problem many men only learn about on forums. The penis stays semi-firm and rubbery when it should be soft, and it often comes with pain, numbness or trouble getting fully hard. Doctors still argue about the cause. In the first published case series, all 4 men said their symptoms began after trauma during sex or tough masturbation. A 2026 review of the cases found that all 4 patients in one report had a history of jelqing, aggressive masturbation or trauma during sex. That is a link, not proof. But it is one more reason not to squeeze a half-hard penis hard and often.
Why Jelqing Is So Hard to Dose
Every other PE method gives you a number. An extender shows the pull, and a pump shows the pressure. With jelqing, the only gauge is your hand. That is why our coaches see so many injuries from it. Men start too hard, add more strokes when nothing happens, and only find out they overdid it the next day.
Your hand has no gauge. An extender or a pump does.
It's very difficult to measure how much pressure you actually applying on your dick and how much pressure you actually need to apply.
Baseem, in his YouTube video on why he does not jelq
Compare the Epic Pro. It shows the exact pull on a screen, and men in our app start at a typical 1.9 kg, about 4.1 lb, in their first week, then build up over months. The Epic Pump shows pressure the same way, and our coaches start men at 3 to 5 inHg and treat 10 inHg as the upper limit. You know exactly where you are, every session.
Signs You Hurt Yourself and When to See a Doctor
Stop right away and rest if you notice any of these. See a doctor, ideally a urologist, if they do not clear up within a day or two, or right away if they are severe.
Pain, numbness or a new bend means stop and see a doctor.
A pop, sudden pain or fast swelling. This can mean a torn blood vessel or tissue. Get checked the same day.
Blood in your urine or from the tip. See a doctor.
Numbness or tingling that lasts. A sign of nerve strain.
A new bend, dent or lump. Possible scar tissue.
Weaker erections that do not come back after a few days of rest.
A penis that stays rubbery and semi-firm when soft.
Our coaches use the same rule for every PE device. Stop at sharp pain, cold, numbness or tingling, and never train through it.
How Jelqing Is Done, If You Still Want to Try It
We do not recommend jelqing, and neither do our coaches. But you will find instructions for it everywhere, and many of them push men to do far too much. If you are going to try it anyway, here is how it is usually described, and what lowers the risk.
The Basic Jelq, Step by Step
Warm up with a warm, damp towel around the penis for a few minutes.
Put a little lube on your hand and the shaft, so your hand slides instead of pulling the skin.
Get partly hard, about half way. Never jelq fully hard.
Make an OK sign with your thumb and index finger and wrap it around the base.
Slide slowly toward the head over 3 to 5 seconds, with light pressure, and stop just below the head, the glans.
Switch hands and start again at the base.
Stop after 5 to 20 minutes, or sooner if you get fully hard or anything hurts.
Dry, Wet and Other Jelqing Techniques
Wet jelqing uses lube, so the hand glides along the shaft. Dry jelqing uses no lube, so the hand grips the skin and pulls it along. Dry jelqing rubs the skin much harder and causes more irritation. If you try it at all, use lube.
You will also see names like V jelqs, squash jelqs and reverse jelqs. They change the grip, the angle or how hard the penis is. None of these techniques has been tested, and the harder versions, done closer to a full erection, put the most force on the tissue. One member in our Discord who did squash jelqs kept bleeding from them.
How Long and How Often Men Jelq
Online guides range from a few minutes to half an hour, and from a few strokes to hundreds. There is no proven routine, so none of these numbers mean anything. Men who logged hand exercises in our app did a typical 10 minutes a day. One member in our Discord found that newbie routines all over the internet called for 100 or more strokes per session, while he got sore after just 10 to 20.
Rules That Lower the Risk
Use less force than you think. Lighter than you would use to masturbate.
Never fully hard. A fully erect penis is the easiest to injure.
Short sessions, rest days. Never two days in a row when you are sore.
Stop at the first warning sign. Pain, numbness, a cold or purple head, bruising or red spots.
Do not combine it with a hard pump session. Stacking force on a swollen penis adds up fast.
Measure honestly. Measure on a rest day, at the same time, the same way, or you will chase swelling.
Jelqing Before and After Results
Search for jelqing before and after and you will find dramatic photos. None of them come from a study, and almost none show how they were measured. Here is what to look for before you believe one.
Taken right after a sessionA pumped or jelqed penis looks much bigger for an hour or two. That is swelling.
Soft vs hardA fuller hang is the easiest photo to take and the least meaningful.
Angle and distanceA lower camera or a closer shot adds size on screen.
No ruler, no dateWithout the same ruler, the same setup and a date, a photo proves nothing.
How Long Does Jelqing Take to Work?
There is no honest answer, because no study shows that it works at all. Men who say they gained talk about months. In our Discord, the clearest measured gain from hand exercises alone took a full year, from 5.6 to 6 inches. For comparison, the typical man on an extender in our app who reached his first quarter inch of measured length got there around month 9.
Are Jelqing Gains Permanent?
The size right after a session is not permanent. It is blood and fluid, and it fades within hours. One of our members gained about half an inch over 3 months of jelqing, and it was gone when he stopped. He thinks it was just better erections. Real change in the tissue, from any method, comes slowly over months and is measured on rest days.

For Length, Use Traction
If jelqing appealed to you because you like the idea of stretching, an extender is the version that has been tested. It holds a steady, gentle pull while you go about your day, and you can see exactly how much. Start at 2 to 3 lb for 30 to 60 minutes a day and build up slowly. Our coaches aim for 1 to 3 hours a day.
The tested version of stretching
Epic Pro, a Steady Pull You Can See
Jelqing guesses at the force. The Epic Pro shows your exact tension and your time on its screen, and the app logs every session. Baseem walks through it in about 5 minutes.

For Girth, Use a Pump
If you jelqed for girth, a pump does the same job, filling the shaft with blood, but with a gauge. Start at 3 to 5 inHg for 10 to 15 minutes in short sets, a few days a week, and use it after your extender, not before.

Epic Pump, Pressure You Can See
Jelqing tries to push blood into the shaft with your hand. A pump does it with a vacuum you can measure. Baseem shows both versions, the Epic Hydro for water or air and the quieter Epic Air.

A Safer Routine That Replaces Jelqing
This is the routine our coaches give new members. It covers both length and girth, every step has a number you can see, and it takes about the same effort as a serious jelqing routine, just with your hands free.
Warm up, 5 minutes. A warm towel or heat pad, then a minute or two of gentle hand stretches while soft. No squeezing.
Extender, 1 to 2 hours. Start at 2 to 3 lb and take a short break every 30 minutes to check comfort and color. Build to 2 to 3 hours a day over months.
All Day Stretcher, optional. 2 to 4 hours of low tension under your clothes after the extender, to add easy hours.
Pump last, 10 to 15 minutes. Interval sets at 3 to 5 inHg in your first weeks, with a full release every few minutes.
Train 3 or more days a week. In our app, men who trained 3 or more days a week were the least likely to lose length.
Log it and measure monthly. The free PE Trainer app logs your sessions and charts your progress. Measure on a rest day.
Want it built around your goal and the time you have? Our free quiz takes a minute and picks the right set for you.

Jelqing Frequently Asked Questions
Does jelqing actually work?
No study has ever measured a size gain from jelqing. Some men say it worked. Of 89 men we read on Reddit, more than half said they gained, typically less than an inch and measured by themselves, and many also used a pump or hand stretching. About 1 in 3 got hurt. What jelqing can do is give you a fuller look and firmer erections for a while, because it pushes blood into the penis. That fades within hours.
Does jelqing increase girth?
There is no study showing it does. The fuller look right after jelqing is blood and swelling. If girth is your goal, a pump does the same thing with a vacuum you can measure. In our app, 72% of men with 50 or more pump hours measured thicker months later, typically +0.24 inches.
Does jelqing make your penis longer?
No study shows that either. A tiny study by a pump company, described by urologist Dr. Rena Malik, found no change in length in 7 men. For length, traction is the method with trials. In one, men gained 1.2 cm of erect length after 6 months on an extender.
What happens if you jelq every day?
Your penis gets no rest, so soreness, bruising and red spots add up. One member in our Discord who jelqed daily kept bleeding from it. Every PE method needs rest days. If you notice pain, a new bend, numbness or weaker erections, stop and see a doctor.
How long should you jelq?
There is no proven answer, because there is no proven routine. The usual advice online is 5 to 20 minutes, stopping if you get fully hard or anything hurts. Men who logged hand exercises in our app did a typical 10 minutes a day.
Is jelqing good for ED?
No. Doctors use pumps and pills for erectile dysfunction, not jelqing. Jelqing can actually cause ED if it damages the blood vessels or nerves. If you have weak erections, talk to a doctor first, because ED can be an early sign of heart problems.
Can jelqing cause erectile dysfunction?
It can. Dr. Rena Malik describes a study of 37 men seen over 11 years who got ED after jelqing. Their scans showed the penis could no longer trap blood well enough to stay hard. Squeezing too hard can also injure the nerves, which causes numbness and weaker erections.
How long does jelqing take to work?
Since no study shows it works, there is no honest timeline. Men who say they gained talk about months, not weeks. The first thing most men notice is firmer erections or a fuller hang, which is blood flow, not growth. Measured gains from any method take months.
Are jelqing results permanent?
The fuller look after a session is not permanent. It fades within hours. One member in our Discord gained about half an inch over 3 months and lost it when he stopped, and he thinks it was just better erections. No study has shown lasting gains.
Is jelqing safe?
It is risky because you cannot measure the force. Reported problems include pain, bruising, red spots, numbness, scar tissue, a bent penis and erectile dysfunction. The Sexual Medicine Society of North America says it is not recommended by most of the medical community.
Can jelqing cause Peyronie's disease?
It may. Peyronie's disease is scar tissue that bends the penis, and it can follow small injuries to the erect or half-erect penis. Doctors warn that jelqing raises this risk, and one man in a published case report developed a deformity after jelqing. If your penis starts to bend, stop and see a urologist.
What is hard flaccid syndrome?
Hard flaccid is a poorly understood problem where the penis stays semi-firm and rubbery when it should be soft, often with pain or numbness. Doctors think it often starts after a penile injury. In one review of cases, all 4 patients reported jelqing, aggressive masturbation or trauma during sex before it began. The cause is not proven.
Is jelqing better than an extender?
No. Extenders have clinical trials behind them for length, and jelqing has none. An extender also sets a steady, measurable pull, and the Epic Pro shows it on a screen. In our app, 90% of men with 250 or more traction hours measured longer, typically +0.55 inches.
Can I jelq before or after my extender?
Our coaches say no. They do not recommend jelqing at all. To warm up, use a warm towel and a minute or two of gentle hand stretching while soft. If you want more blood flow after your extender, use a short pump session at low pressure.
Can your penis still grow after 25?
Not on its own. Natural growth stops after puberty. After that, the only at-home method shown in studies to add length is steady traction over months, like an extender worn most days. Jelqing has not been shown to do it at any age.
Do Kegel exercises make your penis bigger?
No. Kegels train the pelvic floor muscles under the penis. They can help with erection strength and control, which is why doctors suggest them for some men with ED or early ejaculation. They do not change length or girth.
What is the best alternative to jelqing?
For length, an extender. It is the at-home method with clinical trials, and you can see and control the pull. For girth, a pump with a pressure gauge. Many men use both, the extender first and the pump after. Both are safer because you know exactly how much force you are using.
