The short answer

Put the extender on when you are soft. Tape the head, pick a cup that seals without pinching, lock the vacuum, rest the base against your body and set a light pull of about 2 to 3 lb. Start with 30 minutes a day, build to at least 1 hour a day by the end of the first month, and take a short break every 30 minutes. Train most days and take at least one day off.

Stop right away if the head turns cold, pale, purple or numb. Expect better erections in a few weeks and measured length over months. In our app, men measured a year in gained a typical +0.47 inches, and the top 10% gained +1.02 inches or more.

What a Penis Extender Is, and What It Is Not

A penis extender is a traction device. A padded base rests against your body, a cup or strap holds the head of the penis, and adjustable rods between them pull gently forward for an hour or more at a time.

Search for "penis extender" and you will also find silicone sleeves you wear during sex. Those make the penis look longer for a night. They do not change your size. This guide is about traction extenders, the kind used in medical studies.

How Does a Penis Extender Work?

Steady, gentle pulling over many hours tells the tissue to slowly adapt and grow, the same idea doctors use to lengthen bone or stretch skin. It is slow by design. Too much pull for too long cuts blood flow and causes injuries, so the right way is moderate tension, regular breaks and months of consistency.

Erect length

+1.2 cm erect after 6 months

54 men wore an extender 4 to 6 hours a day. Erect length rose 1.2 cm on average, and the gain held for the 3 months men were followed after stopping. Girth did not change.

What You Need Before Your First Session

Set everything out first. A session goes wrong most often in the first 5 minutes, when the cup is the wrong size or the tape is missing.

Flat lay of what you need before a penis extender session: a timer, mild soap, cohesive tape, a towel, a tracking app and a tape measure
Have everything ready before you start: timer, tape, towel and a way to track.
  • The extender with its rods and a tension scale you can read.
  • Vacuum cups in more than one size, with a hand pump.
  • Silicone sleeves to cushion the skin and help the cup grip.
  • Cohesive tape, the kind that sticks to itself, not your skin.
  • A timer, ideally the PE Trainer app, which times and logs each session.
  • A ruler and a notebook, or the app, to record your starting size.

Pick the Right Cup Size

The wrong cup size is the mistake our coaches correct most, about 225 times in our Discord. A cup that is too big lets air leak and the head slip out. A cup that is too small pinches the rim and causes blisters. Epic cups come in 5 sizes, from 28 to 44 mm, so you can try them until one seals snugly without pinching.

Measure Before You Start

Record where you start, or you will never know if it is working. Measure bone-pressed erect length: get fully hard, press a ruler into the fat pad above the penis until it touches the pubic bone, and read to the tip. Measure girth around the middle of the shaft with a soft tape. Do it on a rest day, at the same time of day, every time.

Tracking is super important because your gains are going to be in millimeters.

How to Put On a Penis Extender, Step by Step

This is the vacuum cup method, the one our coaches teach for Epic extenders. It takes about 5 minutes once you have done it a few times.

  1. Étape 1

    Start soft, clean and dry

Put the extender on when you are soft, never erect. Wash and dry the area. Trim pubic hair short around the base, because hair lets air leak out of the cup.

  2. Étape 2

    Pick the right cup size

The cup should seal around the head without pinching the rim. If it slips, try the next size down. If it pinches or leaves a ring, go up a size.

  3. Étape 3

    Tape the head

Pull the penis out a little and wrap Epic Cohesive Tape around the head, starting just below the rim, until the whole head is covered. Firm, not tight. It keeps the head from swelling into the cup and blistering.

  4. Step 4

Fit the sleeve on the cup

Fold the sleeve's edge back over the cup and line it up with the red line. Put the taped head in the cup, then roll the sleeve down over the shaft, flat with no folds.

  5. Step 5

Pump the cup, then lock it

Twist the valve open, give the hand pump one or two squeezes until the cup holds firmly but does not hurt, then twist it closed.

  6. Step 6

Seat the base and hook the cup

Rest the padded base against your body at the base of the penis. Hook the cup into the top of the extender so the device runs straight along the shaft.

  7. Step 7

    Set a low tension

Lengthen the rods until you feel a gentle pull, about 2 to 3 lb on the scale or the Epic Pro screen. You should barely feel it in your first weeks.

  8. Step 8

Start a timer and check in at 10 minutes

Start your session in the PE Trainer app. After 10 minutes, check the head. Pink and warm is good. Cold, pale, purple or numb means take it off now.

The Tape Method

Epic's setup tapes the head every session, under the cup. Start just below the rim and wrap until the head is covered, firm but not tight. It grips itself, so it comes off clean with no pull on skin or hair. Check for pressure or numbness in the first 10 minutes and rewrap if needed. Poor taping is the fourth most common fix our coaches make, so take your time with it.

Strap and Noose Extenders

Cheaper extenders hold the penis with a strap or a noose behind the head. They work, but they squeeze one thin ring of skin, which gets sore fast and can cut blood flow. A vacuum cup spreads the pull over the whole head, which is why most men can wear it longer. If you already own a noose model, keep the tension low and check color and feeling often.

How Much Tension to Use

Start at about 2 to 3 lb, enough that you barely feel a pull. Your goal in the first weeks is to get used to traction and build the habit, not to push hard.

Start by stretching with 2-3lb. You should barely feel it first few weeks and your goal should be conditioning.

Signs Your Tension Is Too High

  • The head turns cold, pale, blue or purple.
  • Numbness or pins and needles.
  • Sharp pain, or soreness that is still there the next day.
  • Blisters at the rim of the cup.
  • Weaker erections. Our coaches say to take 2 days off, then come back with lower tension.

How Long to Wear It Each Day

Start with about 30 minutes a day in week one. Build to at least 1 hour a day by the end of the first month, then 2 to 3 hours as your body adapts, in blocks of 30 to 60 minutes.

In our app, the typical man wears his extender 60 minutes on a training day. 32% of all extender days were 60 to 90 minutes long, 33% were under an hour, and only 9% went past 3 hours.

That last point is the key. Gains climb with total hours logged, not with marathon days. Read the full breakdown in how long to wear a penis extender.

Take a Break Every 30 Minutes

Take the pull off for 5 to 10 minutes every 30 minutes or so. Blood flows back into the head, the skin recovers, and you can check color and feeling. Men who skip breaks get the blisters and numbness that end routines early.

Work in blocks of about 30 minutes, then give blood flow a few minutes back.

Can You Wear It All Day or Overnight?

Not the extender. Never wear it to sleep, because you cannot check how the head looks or feels. For extra hours, our coaches use a low-tension all day stretcher after the focused extender session, while you are awake.

How Many Days a Week

Most days, with at least one rest day. Regular users in our app train a typical 2.3 days a week, and the men who trained more often did best.

The typical gain was about the same, but men who trained 3 or more days a week were about four times less likely to end up shorter. Rest days still matter: tissue adapts between sessions, and "no rest days" is on our coaches' top 10 list of mistakes.

A 12-Week Beginner Routine

This is how our coaches start new members. Move to the next stage only when the current one feels easy, with no blisters, cold or numbness.

WeeksPlan
Semaine 130 minutes a day at 2 to 3 lb, or every other day if you feel sore. Learn the setup and check the head at 10 minutes.
Weeks 2 to 4Build to 1 hour a day, 5 days a week, in two 30 minute blocks with a break. Add tension only if the last week felt easy.
Weeks 5 to 81 to 2 hours a day, in 30 to 60 minute blocks. Most men are near 5 lb by now. Add an all day stretcher for low-tension hours if you want more time.
Weeks 9 to 122 to 3 hours a day as your body allows, one or two rest days a week. Measure once a month, same conditions every time.

After that, aim for 15 to 25 hours a week. Our coaches see gains show up around 250 to 300 hours and get obvious around 500. Under 5 hours a week, results drop.

Comfort Fixes That Keep You Going

Comfort decides who keeps going. When men in our Discord said why they paused, half the reasons were skin problems or a cup that would not stay on. Not one man quit over cost.

How to Stop Blisters

Blisters usually come from the rim of the cup, and they take about a week to heal. Most men who get them fix them the same way: a better fitting cup, smoother taping, a silicone sleeve underneath, and a slower tension build. Let a blister heal fully before you put a cup over it again.

If the Cup Keeps Slipping

A slipping cup means you are losing vacuum. Take the cup off the sleeve and test it on a smooth, hairless patch of skin. If it holds there, the leak is your sleeve or hair: trim short, make sure the sleeve is flat with no folds, has full contact with your skin and covers about two thirds of your shaft. If the cup will not hold vacuum anywhere, contact support for a replacement.

Tips for Uncircumcised Men

103 men asked about this in our Discord. Pull the foreskin back, but not too far, or you strain the attachment. Tape one row just below the head to hold it back, then tape the head. Check that no skin is caught between the cup and the sleeve, because that hurts. Above all, make sure the head is in the cup, so you stretch the shaft, not just the foreskin.

Wearing It Under Clothes

100 men in our Discord asked about wearing it at work or in public. The low-tension all day stretcher is the one built for that. A full extender is bulkier: angle straps can hold it pointing down under loose pants, but most men do their focused extender session at home and the all day stretcher outside.

The 10 Mistakes Our Coaches Fix Most

We read our coaches' messages in our Discord and counted what they correct most often. Here is the list, with the fix for each.

  1. Wrong cup or sleeve size 225 fixes
Air leaks and a head that slips out. The most common fix by far. Try every cup size until one seals without pinching.
  2. Measuring the wrong way 173 fixes
Changing how you measure hides real progress. Measure bone-pressed, erect, at the same time of day, every time.
  3. Low tension for hours, instead of high tension first 157 fixes
Do your focused session with the extender first. Then add low-tension hours with the All Day Stretcher.
  4. Poor taping 151 fixes
Loose tape lets the head slip, tight tape cuts blood flow. Firm, smooth, no folds.
  5. Expecting fast results 95 fixes
Measured length takes months. Most men notice better erections first.
  6. Too few hours 94 fixes
Under about 5 hours a week, results drop. Build toward 1 to 2 hours a day.
  7. Too much tension too soon 84 fixes
It leads to blisters, pain and time off. Add tension slowly, over weeks.
  8. Compression hanger without breaks 72 fixes
If you use a compression hanger, 20 to 25 minutes max, then a 5 to 10 minute break.
  9. Training through pain, cold or numbness 50 fixes
These are stop signs, not a test of how tough you are.
  10. No rest days 36 fixes
Tissue adapts between sessions. Take at least one day off a week.

Counts are approximate, from coach messages in our Discord since 2023.

Is It Safe? Side Effects and When to Stop

"Is it safe?" is the number one question new members ask, 233 men in our Discord. Used the right way, traction is low risk. In the first randomized trial of a traction device, side effects were mild and did not last.

The problems men do report almost all come from too much, too soon. Blisters, mostly from the cup rim, were the most talked about. Pain and soreness came next. Swelling and red spots were mostly from pumps, not extenders. Weaker erections were rare, and most of those messages were men worrying, not reporting it.

Stop Right Away If You Notice This

Take the extender off at once if the head turns cold, pale, blue or purple, if it goes numb or tingles, or if you feel sharp pain. Rest, let the color and feeling come back, and restart another day with lower tension and more breaks. If color or feeling does not come back quickly, or you see blood, see a doctor.

Cold, numb, color change or pain means stop now.

If you feel too much discomfort or pain, STOP IMMEDIATELY. Take a break, lower tension significantly and make sure you don't get to that level again.

Talk to a doctor before you start if you have a bleeding disorder, poor blood flow, diabetes with numbness, or any recent surgery or injury in the area. If you have a curve, read penis extenders for Peyronie's disease first.

When Will You See Results?

The first change is usually not length. Men in our Discord noticed something after a median of 4 weeks, most often harder erections or a longer flaccid hang. Measured erect length comes later.

How Long Does It Take To See Results?Baseem on YouTube · 6 min. Why real gains take months, and why tracking matters.

It's not that enlargement doesn't work. It's that men don't work long enough to let biology catch up.

Baseem, on YouTube

The hard part is staying with it. In our app, 52% of extender users were still training after a month, 30% after 6 months and 21% after a year. The men in that last group are the ones with the big numbers. See their photos in penis extender before and after and every chart in the Epic Traction Report.

Should You Add a Pump?

If you want girth, yes. An extender barely moves girth: men who only used one had a typical girth change of 0.00 inches. In our app, men who also pumped for 10+ hours gained more length too, a typical +0.39 inches vs +0.24 inches without a pump. Pump after the extender, not before. Read how to use a penis pump.

How to Use the Epic Extender

If you own an Epic, start with the setup video on our Epic Extender manual page, then open the free video course in the PE Trainer app. The steps above are the same ones the course teaches.

Getting Started With the Epic Extender

  • Week one: 30 minutes a day at low tension, on the printed scale on the rods.
  • By day 30: at least 1 hour a day, then 2 to 3 hours in blocks of 30 to 60 minutes as your body adapts.
  • Cups: twist the valve open, pump, twist it closed to lock, and twist to release when you finish. The Epic Cups V3 seal holds for hours, so you should not need to re-pump.
  • Tape: Epic Cohesive Tape covers the head under the cup and helps prevent blisters.
  • Sleeves: they wear out. Replace them when they thin, tear or stop gripping.

Cleaning and Care

Wash the cups and sleeves with mild soap and warm water after every session, rinse well and let them air dry. Store sleeves flat. Wipe the base and rods. Use fresh tape each time. Check the cup valve and the rods now and then: if a cup stops holding vacuum or a rod stops threading smoothly, replace it. A clean setup grips better and is kinder to your skin.

Penis Extender Questions

How do you use a penis extender for the first time?

Put it on soft, tape the head, pick a cup that seals without pinching, lock the vacuum, rest the base against your body and set a light pull of about 2 to 3 lb. Wear it for about 30 minutes, check the head after 10 minutes, and stop if it gets cold, pale, purple or numb. Build up slowly over the next few weeks.

How many hours a day should you wear a penis extender?

Start with about 30 minutes a day in week one, build to at least 1 hour a day by the end of the first month, then 2 to 3 hours as your body adapts, in blocks of 30 to 60 minutes. In our app, the typical man wears it 60 minutes on a training day. Men who wore it 45 to 90 minutes a day gained as much as men who wore it longer. See our full guide on how long to wear a penis extender.

Can you wear a penis extender while sleeping?

No. You cannot check for cold, numbness or color change while you sleep, and the cup can shift. Our coaches never recommend overnight wear. For long, low-tension hours, use an all day stretcher while you are awake.

Do you put on a penis extender hard or soft?

Soft. An extender works on the flaccid penis. If you get an erection during a session, take it off and wait until you are soft again.

How much tension should a beginner use?

About 2 to 3 lb, enough that you barely feel it. In our app, Epic Pro users wore a typical 4.1 lb in their first week and 5.9 lb by week 13, so tension rises slowly over months, not days.

How long does it take to see results from a penis extender?

Most men in our Discord noticed something within about 4 weeks, usually better erections or a longer flaccid hang. Measured length takes months. In our app, the typical man reached his first quarter inch around month 9, and men measured a year in gained a typical +0.47 inches, with the top 10% at +1.02 inches or more.

Does a penis extender increase girth?

Not much. Studies found no real girth change, and in our app men who used only an extender had a typical girth change of 0.00 inches. For girth, add a pump. Read how to increase girth.

Are penis extender gains permanent?

Gains from slow traction appear to last. In the one study that measured erect length, men kept their gain for the 3 months they were followed after stopping. Swelling right after a session is not a gain. Measure on a rest day.

Is it safe to use a penis extender every day?

Most men do best with most days on and at least one day off. In our app, men who trained 3 or more days a week were the least likely to lose length (2% vs 10% under 1.5 days a week). In a controlled trial, side effects from traction were mild and did not last.

Can you pee with a penis extender on?

No. Take it off first. Putting it back on is quick once you know your setup.

Can you wear a penis extender to work or under clothes?

Many men wear a low-tension all day stretcher under clothes at work. A full extender is bulkier. Angle straps can hold it pointing down under loose pants, but most men do their focused extender session at home.

Why does my penis keep slipping out of the extender?

Usually the cup is the wrong size or air is leaking. Test the cup alone on a smooth, hairless patch of skin. If it holds there, the leak is your sleeve or hair, so trim short and smooth the sleeve. Make sure the sleeve covers about two thirds of your shaft.

Why is the head of my penis cold or purple?

The pull is too strong or the session is too long, so blood flow is cut. Take it off right away. Next time lower the tension and take a 5 to 10 minute break every 30 minutes. If color or feeling does not come back quickly, see a doctor.

Can uncircumcised men use a penis extender?

Yes. Pull the foreskin back, but not too far, before the cup goes on, and tape one row just below the head to hold it there. Check that no skin is caught between the cup and the sleeve, because that hurts. Make sure the head is in the cup, so you stretch the shaft, not just the foreskin.

Is a vacuum cup or a strap better?

For most men, a vacuum cup. It spreads the pull over the whole head, so it is more comfortable for long sessions. A noose or strap is cheaper but squeezes one ring of skin and can cut blood flow.

Should I use a pump before or after the extender?

After. Our coaches put the pump at the end of the day, after the extender and the all day stretcher. Read how to use a penis pump.

Do I need to shave to use an extender?

No, but trim short around the base and the shaft. Long hair lets air leak out of the cup and can get pulled under the tape.

How do I clean a penis extender?

Wash the cups and sleeves with mild soap and warm water after each use and let them air dry. Wipe the base and rods. Use fresh tape every session. Replace sleeves when they thin, tear or stop gripping.

Where are the Epic Extender instructions?

The setup video is on our Epic Extender manual page, and the full video course is free inside the PE Trainer app. You can also ask Baseem's coaches in our Discord.

Does the Epic Pro connect to an app?

Yes. The Epic Pro sends your tension and session time to the PE Trainer app over Bluetooth, so every session is logged on its own. The app also reminds you to train and to measure.
